Vendor Contractor Affidvait Agreement

Dear Pike County Vendor,
New regulations will go into effect with the start of the New Year, 2010 that impact business transactions with public government entities. One of these is the implementation of the Immigration Reform and Control Act of 1986 (IRCA) and the Georgia Security and Immigration Compliance Act OCGA 13-10-90 (SB 529), or the E-Verify program. The State of Georgia, with SB 529, has mandated that all government agencies and public employers participate in this program. Compliance with this new regulation will require that all vendors providing services or the possibility of providing services to Pike County complete the Vendor/Contractor and Sub-contractor affidavits for permanent record. The County is requesting that a Vendor Registration form be completed for our files along with these affidavits, even if you have completed a similar one in the past. This form will be used to compile a vendor database for record retention and for notifications of future bid opportunities.
The new Vendor Registration form and the vendor/contractor affidavits are available on the County's website, in order to make this process as simple as possible. Just click on a link above to download the appropriate form, fill out the form online and print it out. Note: these are Adobe Acrobat files and you will need to have Adobe Reader on your computer to view them. If you do not have this program you may download a free copy. The affidavits require a 'fresh ink' signature and notarization. The Vendor Registration Form does not need to be notarized. All forms need to be mailed to:

This requirement goes into effect January 1, 2010. These E-Verify affidavits will need to be on file with our office as soon as possible. Once the notarized affidavits have been received and processed, a notification will be sent to the email provided. If you are not able to access our website to complete the registration for your business, please request these forms to be mailed by sending an email with the business name and address to email@example.com All submitted forms (with the exception of signatures) must be typed in order to maintain OCR compatibility. If you wish to no longer be considered a vendor for the Pike County, please send an email to that effect to the address above. If you know you are exempt from the E-Verify process, please note that on the affidavit, sign and return as well.
